Disclaimer: Due to the highly infectious nature of the Omicron Strain of COVID-19, a lot of testing sites are overwhelmed with long waiting lines. To facilitate and offer convivence to our local community, Fusion Muslim Community Center has decided to offer its parking lot for COVID-19 Testing. The tests are collected and test results are provided by an Illinois based testing lab. Fusion is not collecting the specimens and thus cannot provide the results. Please contact the lab directly for follow-ups.

This is a drive through testing site and you do not have to exit your vehicle. Fusion facilities or its restrooms will not be available for use. Thank you for your cooperation 

Due to the overwhelming COVID-19 cases and increased testing, the lab will provide the test results via email in 48-72 hours.  Please follow the complete instructions provided below and complete all requirements before coming to Fusion. If you arrive at Fusion without completing this form, you will need to park and complete the registration form on your mobile and obtain the specimen ID before the testing can begin. Please make sure to bring a state ID.  

INSTRUCTIONS:

PLEASE FOLLOW THE INSTRUCTIONS PROVIDED HERE FOR A QUICK RETURN OF YOUR TEST RESULTS

1. Click on the link: https://forms.oclsolutions.com/212796707190967
   (Note: This link will take you to a 3rd party website, keep Fusion website open for instructions)
2. When the registration form opens, under “Select Location”, pick “syed quadri[8703 quadri] “ from the drop down
3. Enter Patient information, Address, Insurance information etc. If no insurance, select “No Insurance” from the drop down
4. Under “Test Selection” check “SARS-COV-2, PCR [COV19]”
5. Under Patient Signature, please sign the form provided.
6. Read the DISCLAIMER provided by the lab
7. Drag and drop Insurance Card if available and your State ID. If submitting for a minor, their passport identification page
8. Click Submit after attaching the documents
9. After submission of the form, you will get a Specimen ID. Please save this ID as this will be required at the time of specimen collection at Fusion. The Lab tracks your specimen and results based on this Specimen ID. Please take a picture and save it. 
10. When you arrive at Fusion to get tested, you will fill out the below form that will be provided. The above Specimen number will need to be entered on this form. Please print all information clearly to avoid delays in results
